Teleholistic offers web-based service, which provides evaluations and management of psychiatric conditions in adults, holistic care, diet, weight management and exercise. It is strongly recommended that you carefully read the folllowing terms and conditions as these terms apply to any interaction you have with Teleholistic staff, through the website, over the phone, during video conferences and through any other method of communication. These terms and conditions are subject to change at any time, at the discretion of Teleholistic and without notice to you. All changes made to these term and conditions will be binding on you. It is your responsibility to review these terms and conditions to remain informed and compliant with the requirements.
Disclaimer of Service
The service provided by Teleholistic is designed to be remote diagnostic evaluations and treatment, and it is different from the in-person services typically performed by practitioners. Practitioners providing services using remote connection may not have the same type or amount of information that would normally be available during an in-person visit with the patient. These are aspects fo the practice of medicine that sometimes require a practitioner to personally examine and observe the condition of the client. Teleholistic providers may not be aware of facts or information that would affect their opinion of your diagnosis. Therefore, a diagnosis is limited by the nature and amount of information available to Teleholistic providers. By engaging in these services you acknowledge and agree that you understand this limitation and agree to assume the risk of any such limitation.
By using the service provided on this website and by engaging Teleholistic providers for services, you understand and agree that:
The lack of an in-person evaluation may affect Teleholistic provider’s ability to obtain information important to your diagnosis and therefore affect Teleholistic providers diagnosis and management of your condition.
By receiving services, you acknowledge and assume the risk of the above described limitation. You further agree that you understand Teleholistic providers have not given you any warranty or guarantee concerning any particular result, cure, or diagnosis.
Privacy and Security
The use of this website is governed by a Privacy Policy. A copy of this policy is posted on the website. Transactions through this website are protected by encryption software to protect the integrity of your personal information. Information sent over the internet outside of the Teleholistic providers control is at risk of being disclosed at any number of points of contact with computers outside of their systems. You must safeguard your own confidential information to the best of your ability. Do not send sensitive information over the internet if you are not satisfied with the level of security offered.
You assume the risk of any undesirable disclosure or unauthorized intrusion resulting from any security breech, and the risk of any interruption or failure of data while using services provided by the Teleholistic provider.
If you choose to provide Teleholistic providers with personal information via email or mail, or by filling out a form and submitting it through the website, he will use that information only for the purposes of providing services. He does not give, share, sell, or transfer any personal information to a third party unless required by law, or unless you provide Teleholistic providers written authorization to do so.
In addition, the Teleholistic providers regard all your personal information and medical records as confidential and takes appropriate safeguards as required by state and federal law to protect the privacy of your confidential information. Teleholistic’s Medical Information Privacy Policy is also posted on this website. Sessions may not be recorded in any format.
Remote services with a Teleholistic provider is primarily conducted via Zoom, FaceTime, Skype, Cisco Webex, Instagram Video, Facebook Messenger or WhatsApp. Please review the Terms and Conditions and Security Policies of these companies prior to agreeing to their services.
Storage of Medical Information
Teleholistic providers will store and securely maintain your personal and medical information in accordance with the requirements of state and federal laws.
Insurance Coverage
The services provided by the Teleholistic providers may not be covered by your personal health insurance, Medicare, or Medicaid, and the Teleholistic provider will not file any insurance, Medicare, or Medicaid claims. Your engagement in the services provided will be considered an understanding of this restriction and your agreement that you are responsible for all payments.
Payment Policy
All payments are made online at the time an appointment is scheduled. Payments are processed through Square.
Refunds
Missed appointments or cancellations made with less than a 48 notice are non-refundable.
Termination Policy
Patients are under no obligation to continue treatment should they decide to terminate at any time. However, if you wish to end treatment prematurely, you are strongly encouraged to discuss your thoughts openly.
Emergencies & Phone Policy
Should you wish to contact the Teleholistic provider between scheduled sessions, you are encouraged to call or email. Teleholistic providers will make every attempt to return messages within one business day.
Should you experience a life-threatening emergency, call 911 for assistance. You should then notify the Teleholistic provider. Every effort will be made to return your call promptly.
No Warranties
The Teleholistic provider makes NO REPRESENTATIONS or WARRANTIES of any kind, EXPRESS or IMPLIED, regarding the use of this website or of the services provided.
The content of this website is provided “AS IS” and “WITH ALL FAULTS” and your use of this website or engagement of the services of the Teleholistic provider confirms your understanding and acceptance of these terms.
The Teleholistic provider, to the fullest extent permissible by law, disclaims all warranties, express or implied, including but not limited to, implied warranties of merchantability, fitness for a particular purpose, title and/or infringement.
Intellectual Property
The contents, selection and arrangement of this website are owned by Teleholistic and are protected by copyright, trademark and other intellectual property laws. Any reproduction, transmission, publication, copy, republish, frame, alteration, distribution, modification, or the creation of derivative works, or other use in whole or in part in any manner of this website or the services provided by Teleholistic without prior written consent is strictly prohibited and may violate these laws.

Teleholistic or it’s providers shall incur no liability for any damage resulting from loss of use, data, or profits, whether or not Teleholistic has been advised of the possibility of such damages, arising out of or in connection with the use of this website, its content, services, or any statements or actions of any third party on or through this website, or any services available, referenced or linked to from this website.
State Jurisdiction
This website (excluding linked websites) is controlled by Teleholistic. This Terms of Service Agreement shall be governed by and construed in accordance with the laws of the State of Nevada, Idaho, Maryland and District of Columbia without regard to its conflicts of laws rules. Any dispute shall be brought in the courts of either of those locations.
Consent to Treatment & Guarantor of Responsibility
By clicking below and using the services of this site, you acknowledge that you have read the Terms and Conditions listed, and you understand and agree with them. You hereby agree to be treated by the Teleholistic providers, and when necessary, other practitioners covering in a provider's absence. You authorize the Teleholistic provider to provide information concerning my treatment to any practitioner or therapist who referred you to this practice. You agree that you are responsible for all charges for services rendered.
